Hydrochloride #Each vial contains: for Injection, USP Vancomycin hydrochloride equivalent to 500 mg vancomycin. 500 mg Usual adult dosage: 2 q daily in divided www.fresenius-kabi.com/us per vial doses. Dilute with 10 mL Sterile Water 3 NDC 63323-221-33 for Injection. For intravenous use. EXP MM-YYYY Prior to reconstitution, Store at Lake Zurich, IL 60047 Lot xxxxxx MUST BE FURTHER DILUTED 395015-A 20' to 25 C (68 to 77'F) [see USP Made in Denmark BEFORE USE. Fresenius Kabi Controlled Room Temperature]- 403370 Sterile, Lyophilized. After reconstitution, this vial may be Preservative free. Rx only stored in a refrigerator for 96 hours. PREPARATION AND STABILITY At the time of use, reconstitute by adding either 10 ml of Sterile Water for Injection to the 500-mg vial or 20 ml of Sterile Water for Injection to the 1-g vial of dry, sterile vancomycin powder. Vials reconstituted in this manner will give a solution of 50 mg/ml. FURTHER DILUTION IS REQUIRED. After reconstitution, the vials may be stored in a refrigerator for 14 days without significant loss of potency. Reconstitute solutions contained 500 mg of vancomycin must be diluted with at least 100 ml of diluent. Reconstituted solutions containing 1 g of vancomycin must be diluted with at least 200 ml of diluent. The desired dose diluted in this manner should be administered by intermittent IV infusion over a period of at last 60 minutes. ORDER: Vancomycin 0.45 g IV q 8h 1.
