When completing the vignettes, there is a vignette assumption. That is, when reading assume that a full assessment has been done and no mention means 'no evidence'. This is the standard quality assurance if it is not documented it does not exist assumption. What that means, then if there is no mention of a need, the rating should be a '0'. If there is no mention of a strength, then the rating should be a '3'. So, no mention of any suicidality would result in a rating of '0' on Suicide Risk. No mention of any talents or interests would result in a rating of '3' on Talents/Interests. Another way to think about it is that you start the assessment with all 0 ratings on the Needs and all 3 ratings on the Child/Youth Strength and then you look for evidence to adjust your scores accordingly using the action levels. Caregiver ratings use the Need action levels for the vignettes. Ordinarily certain items would have an N/A option (for example Job Functioning for an underage child, or Foster Caregiver for a child not in foster care). For the purposes of this test please rate such items as '0' for needs and as '3' for strengths. Good luck!

Oliver is a 39-year-old veteran who lives with his wife and 6-year-old daughter. Oliver reports that he hears voices and sounds every night while trying to fall asleep. Worry that these sounds will keep him awake has led to disturbed sleep and an inability to get the amount of sleep he needs to function well the next day. He reports not currently having suicidal thoughts, but he has within the past six months. Oliver retired from the military 18 months ago. While in the military, he was deployed on four different occasions to war zones and reported having nightmares about his experiences there. He will not go into detail about them, saying it is too painful and doesn't want to talk about it. Oliver's wife Laura is very concerned because he is not getting adequate sleep and is also having a difficult time controlling his anger because of these experiences. Laura says that he has been taking his anger out on her and their daughter; although he has never physically assaulted them, he is often verbally abusive. The verbal abuse has worsened to the point where their daughter is now afraid of Oliver. Laura says their daughter does not want to be alone with him because she is fearful of him yelling at her. Last night his daughter screamed when he tried to pick her up, and Oliver realized he had a problem and sought help at the VA so that he doesn't escalate to violence. While he generally makes good decisions, he has been struggling recently because of his volatile behavior. Oliver's parents they live about an hour away. His father is a minister and he grew up attending church regularly. He frequently tells his wife that he feels that he is being punished for not being as religiously active as he should be, although he does attend services periodically. Oliver reports that he has a fairly decent relationship with his parents but wishes he could spend more time with them. However, he has difficulty going back to his childhood home because of haunting memories. While a senior in high school, his twin sister took her own life. Oliver was the one who found her after he got home from football practice. He believes that if he hadn't stayed after practice to work out, he would have been home earlier and could have saved her. Oliver joined the military two weeks after his high school graduation needing to get away. Oliver knows several fellow veterans that he considers close friends, and he joins them for game night once a month. He also really enjoys reading because it allows him the opportunity to escape reality and to venture into an alternative universe. He is also very involved in his community where he and Laura volunteer three days a week at the local homeless shelter. He has been unemployed since retiring from the military but has been thinking about what is next for him, and wants to attend school to become a barber. He learned to cut hair while he was deployed and enjoys the thought of owning his own shop one day. Oliver has the school application but admits that he cannot find the motivation to actually complete it. Laura has expressed a willingness to help, but he has refused the help, stating that he wants to work on getting his mind in a good place first before having to be around other people all day in school.
