Create a safe physical environment and institute general fall prevention interventions. Key Outcomes 1. Reduction in fall rates 2. Reduction in injuries resulting from falls 3. Reduction of repeat falls

Fall Assessment and Documentation, per Patient Type Inpatient: Fall risk assessment must be completed and documented within 3 days of admission, upon transfer to another level of care, or when a change in condition occurs. Emergency: Fall risk assessment must be completed and documented within 3 days of admission. If at risk, will be assisted with elimination and mobility needs, and other patient-specific interventions will be initiated.

Surgical: Fall risk assessment must be completed and documented within 3 days, upon transfer to another level of care, or when a change in condition occurs. Medication Profile Review and Documentation All patient medications must be reviewed for appropriateness by a nurse or doctor within 24 hours of admission.

Vital Signs and Documentation All patient vital signs must be taken and documented within 24 hours of admission.

Physician Office Practices Patient will be assessed for fall risk as appropriate to the reason for the office visit, utilizing encounter forms to evaluate medical history and problems (medications; history of falls; visual, hearing, gait, and balance impairments; medical conditions). Documentation of any findings and recommendations will be noted in the medical record on the appropriate forms.

Communicate Risk Communicate a patient's risk for falling to the healthcare team and family/support system as appropriate for setting (e.g., for inpatients: "Fall Risk" ID bracelet and "Fall Precautions" sign, handoffs, mobility cards, whiteboards).
