Quality and Safety Management In order to evaluate the current state of health care and make effective and important improvements in patient quality and safety, it is critical to identify meaningful performance measures. It is important that these measures can be applied in the same way in similar health care institutions, wherever they are. As noted in your course text, there are three types of performance measures used to evaluate health care: structure, process, and outcome. In this discussion, you create and evaluate effective measures and apply them to the case study you identified in Week 1. To prepare for this Discussion: Review the Learning Resources, specifically Chapters 3 and 4 in your Course Text, Introduction to Healthcare Quality Management, to find examples related to performance measures and how to present the data collected. Post by Day 4 a brief description of three different performance measures you would create (one each for structure, process, and outcome) that could be applied to the case study you identified in Week 1. Then, indicate the types of data you would need to collect to support the measures and how you would analyze and present that data. Finally, identify improvements you expect might result from your analysis. Support your work with specific citations from this week's Learning Resources and/or additional sources, as appropriate. Medical Error

Newly born babies are generally mistreated because of common name problem like baby girl or baby boy. At a time several babies are born in a hospital and the process of giving same name at their batch is a big problem, as often babies are lost or wrong treated (Waiting to pick your babys name raises the risk for medical (NPR), ( n.d).

IOM six dimensions of US health care give a proper guideline to treat the patient for utmost benefit. Six dimensions are given as follows:

• Safe: Medication or treatment given to patient should be safe which require proper details about the patient. If patient is misidentified then he or she will be mistreated.
• Effective: Effectiveness of the medical services will be affected if given to a wrong patient as all the babies look similar after birth and it becomes highly difficult to remember the patient.
• Patient Centered: Medication should be given only after proper analysis of the patient as every patient is different but a file containing wrong baby name can be problematic.
• Timely: Treatment to the needy baby might not be given on time if he or she is misidentified.
• Efficient: Efficiency of the staff is highly affected as a wrong treatment will create a great mess for both the babies.
• Equitable: Equal treatment or every gender or race should be given.

In the above case the most significant dimension is "Time" and "Efficiency", as if the parents are timely informed about the mess which can happen they can decide a name before delivery. And, if the staff could have been efficient then they would have already thought a way for it (Waiting to pick your babys name raises the risk for medical (NPR), ( n.d). The medical error here could have been prevented by adopting a naming system, medical organization could have already get the babies name on paper before delivery, one for baby girl and other baby boy, optional names could be taken in case of twins.
