For each recommendation, the panel took a population perspective and came to consensus on the following: the certainty of the evidence, the balance of benefits and harms of the compared management options, and the assumptions about the values and preferences associated with the decision. The guideline panel also explicitly considered the extent of resource use associated with alternative management options. The panel agreed on the recommendations $($including direction and strength$),$ remarks, and qualifications by consensus or$,$ in rare instances, by voting $($an $80\%$ majority was required for a strong recommendation$),$ based on the balance of all desirable and undesirable consequences. The final guidelines, including recommendations, were reviewed and approved by all members of the panel. The approach is described in detail in the accompanying article describing the methods of development. The recommendations are labeled as either strong or conditional according to the GRADE approach.
