Maintenance contract Summary Correlation Table This Maintenance Contract is entered into on 06-05-2023 between pizza Slice (Owner), and Moka maintenance (Contractor), whose official business address is 321 main street ,Snina 123-567-1122 moka@maintanance.com This contract applies to the property located at the following address: 123 Main Street, Snina. The owner of the property listed above wishes to hire Contractor for the purpose of maintenance and repair services for the pizza ovens, mixers and refrigerator(collectively referred to as the ("Equipment") Contractor Duties The parties agree the contractor will perform the following duties: 1. Contractor must provide regular maintenance on all Equipment 2. Contractor must perform repairs on Equipment, included but not restricted to inspection, cleaning, and replacing of worn parts as required. 11A:9.50. Maintenance contract, 6 N.J. Forms Legal \& Bus 11A:9.50 3.Notifying owner of repairs that require an investment of capital within 2 days. 4.Performing other routine maintenance as needed and requested by owner. Contractor is permitted to hire sub-contractors to satisfy the terms of this contract. Owner's Responsibilities and Duties Owner will provide Contractor with access to the property, including giving Contractor any keys or security codes needed for access. Owner will pay Contractor on a bi-weekly basis at the daily rate of $50 an hour. The owner agrees to pay an hourly rate of $50 an hour for all additional work not covered in this contract. Contractor is an independent contractor and responsible for paying all employment and state and federal taxes. Owner wi not withhold any amount from Contractors pay for paying taxes. Inswramce toris isponsible for securing liability insurance and for submitting proof of liability coverage before beginning work. 2. As an independent contractor, Contractor is not entitled to worker's compensation. Contract Termination This contract may be terminated at any time, by either party, without penalty, provided a 14 day notice is given in writing. Owner may cancel the contract at any time, without advance notice or penalty, in the event of gross negligence on the part of Contractor. By signing below, both parties agree to be bound by the terms of this contract
