The Passage

Dr McCarthy is a general practitioner who practise in a small office in Umhlanga. Dr McCarthy practice is faced with issues such as space and storage from lack of space to accommodate patients to no storage for his medical supplies. Amongst his storage and space challenges, Dr McCarthy is mostly worried about poor management of patient's records. These records are stored in cabinets in the practice, taking up more space and furthermore with no security measures. Despite security risks, these records can not be accessed easily when needed. It is also very difficult to share these records with appropriate team entities when needed due to the difficulty in accessing these records. Some of these records are misplaced and they are also prone to errors, duplicates, and inconsistencies.

The Question

Design a logical structure by defining the patient, doctor, and finance tables and after that define a data dictionary for the tables for each one.

You should do a simple Entity Relationship Diagram using the structure in your logical structure to apply rules such as primary keys, foreign keys and normalization.
