Select any three (3) hazards or unsafe conditions that I have observed on site.
Describe how selected hazards could harm workers; refer to Injuries & diseases - WorkSafeBC to search for possible harm from various hazards.
Assess the risk for each of the hazards using the Risk Level table presented below.
Recommend the appropriate controls; refer to Hazards & exposures - WorkSafeBC to search for various controls for specific hazards.

Here is an example of the hazard observed on site:
Hazard 1:
Uncovered trench
Harm:
The trench is long and deep, which may cause falls and injuries to occur. If trenches are very deep, workers can fall into them and then be trapped inside.
Can cause slips and trips as well when working in area or stepping of machines/equipment.
If not covered, then natural weather, such as rain, may become trapped and overflow into the work areas which may increase hazards and risks for injury.
In addition, digging trenches may gain access to anything installed underground, such as phone lines or electrical lines and if not covered, may cause more injuries from the damaged lines.
Assessing the risk:
Impact: moderate most common injury is falling down. Since it is at the ground level, the injury should not be significant unless it is deep, then it can be a medium.
Likelihood: likely trenches are created temporarily until filled.
Overall Risk Level: Medium
Controls
Place a temporary yet fixed (not easily movable) cover over the trenches so employees can walk on it, or guarded by fixed or movable guardrails.
Place caution sign to notify workers
