Instructions

1. Choose a health behavior, such as exercise, or a behavior that is risky to a person's health, such as smoking cigarettes. You may use theTheory of Planned Behavior Model form as a guide, or create your own diagram using any of the tools listed as follows:

Label the five bubbles with the concepts explained in the course video or the labels from the Theory of Planned Behavior in your textbook.In the "comments" (right click each bubble) for each bubble,write howeach concept/component applies to your chosen behavior of interest.
  • Web-based Users:You can download the TPB-model document and fill in the appropriate parts of the model. When you have responded to all the questions in the worksheet, scan and save the file with your initials added to the filename (e.g. TPB-model_SL.docx) and upload your edited file to the assignment box in Canvas.
