You are expected to submit a report (in a Word document) at the end of the course following the sample format provided. The report should be a minimum of 5 pages.
Sample Format
Title/Cover Page:
Include the title of the report, your name, who you are submitting it to, and the date.
Purpose:
The purpose is a statement that identifies whether the issue is a new situation that has come to light and needs to be addressed/remedied or an ongoing issue that needs to be addressed. It also includes who and what has precipitated the report.
Example:
This report examines/addresses _______, which needs to be resolved because _______. The recommendations outlined at the end of this report address/correct the identified issues. This report is intended for _______ to authorize the implementation of the recommendations.
Use the SMART principle when creating the purpose: Specific, Measurable, Achievable, Relevant, and Time Sensitive.
Key Points:
The paragraph of key points follows your purpose and includes the main points to be discussed regarding the problem/situation. It identifies the sections into which the materials are organized and the proposed recommendation.
Example:
Given _______, this report will address _______ and the recommendation _______ to resolve the issue. This report will review _______.
Discussion of Findings:
Review your key points in detail, including facts, figures, charts, and your analysis of the problem. There should be one key point per section/paragraph. Arrange your points in the order of their importance to your purpose. They should lead logically to your conclusion/recommendations. Use headings and subheadings to divide the report into the relevant sections. Use a chronological order.
Example:

Conclusion:
The conclusion summarizes the report, presenting the recommendations, which should include a timeline, cost breakdown, and measurables.
