You are to design a database to keep track of patient hospital visit records. Any patient can have any number of hospital visit records at the hospital. A patient does not have to have any visit records. Any doctor can treat any number of patients. A doctor does not have to treat any patient. A visit record has to be associated with exactly one patient AND one doctor. Any visit record can have any number of treatments, i.e. flu shot or EKG, etc. Any treatment can be associated with any number of visit records For each patient, you need to record patient's frst name, last name, a phone number, and the unique patient id that never changes over the patient's life time. For each doctor, you need to record a unique doctor id, first name, last name, and department. For each treatment, you need to record the treatment's unique id, name, and cost. For each hospital visit record, your need to record the unique visit id, date, and who the patient and the doctor are. You also need to record the treatment information that is associated with each hospital visit record, if any. Additional Requirements 1. If you realize the requirement description lacks some logical details, please use your best judgement to make appropriate assumptions. You have to write ALL your assumptions in the Word file that you submit. Your assumptions have to be consistent with your ERD 2. Your ERD and your database schema have to be consistent in EVERY logical aspect 3. Your ERD has to specify cardinalities such as One or Many, Mandatory or Optional Your ERD has to specify Primary Key(s) in each and every entity. But your do NOT have to specify Foreign Key(s) in an entity Your data schema has to specify ALL attributes in all entities, including Primary Key(s) AND Foreign Key(s) 4.
