How do you measure your own work in a project? What quality standards do you attempt to meet?

To what extent is an effort in place to define tasks with risk and tasks to which project management attention is directed? Or, alternatively, is each project manager left to his or her own devices to figure out how to review the work?
How is the management of nonrecurring, exception, workaround, and rework effort managed? Is it handled differently from routine tasks?
