Instructions

The clinic is conducting an improvement project to improve efficiency. Your task is to gather data. You must assist the project team collect information on patient flow in the clinic. The team wants to

know what time a patient arrives at the clinic and what time they leave. In addition, they want to know how long it takes to complete activities that occur during their appointment. You ask the team for

more specifics so you are sure the information gathered will meet their needs. The team states they want to know the average time between various steps in the clinic process. You determine the

following data elements should be gathered on each patient during the study time period:

Time of the patient's scheduled appointment

Time patient leaves home to travel to the clinic

Time patient arrives at the clinic

Time patient checks in with receptionist

Time patient is shown to the exam room

Time nurse finishes clinical measurements $($e$.$g$.,$ patient's weight, height, lung function$)$

Time each CF team member comes into exam room to see patient and time they leave $($CF team members include: physician, nurse, social worker, nutritionist, physical or respiratory therapy, and

others$)$

Time patient is ready to leave the clinic
