In any program, bid data is key to determining success and guiding sustainment.  Without metrics proving success would be impossible.  Consider the Access to mental health  telehealth program you propose, along with the health disparities you aim to address and research the quality and other data metrics you will need to collect to report to your key stakeholders.  

• At least 4 data metrics to be collected for reporting needs
• Why are these areas you selected intriguing or of interest? 
• How will this data impact social determinants of health? 
• Discuss how you will model the data to tell the "story" of the status of the program
• Discuss the benefits of collecting this data, and who is the primary stakeholder of the data. 
• Create recommendations for process improvement to ensure the sustainability of your program
