The team knows what goals they want to achieve as identified in Step $1,$ and will evaluate the process to determine what are the main problems to address in Step $2.$ Now the team has to make the changes and carry them out to see if the process improves.

Improvement Method: The QI Team will create a plan for the improvement project and identify the change they want to make. The team will then implement this change on a small scale to test it out first. They will then gather data and study the results if the change made an impact. If the desired changes did not occur, they will revise the improvement process and start over again. If the desired changes did occur, the change will be implemented on a larger scale.
