For each team, you must choose one member to be the team champion and four others. Each team must meet the following criteria:

All teams must have a minimum of 5 members. No individual can be assigned to more than two teams. Issues related to the diversity of members backgrounds and experiences must be considered. You must justify your selection of each member for all three initiatives.

1. Initiative A: Reducing Maternal Mortality for High-Risk Communities Goal: Reduce the maternal mortality rate by 25% over the next five years in three high-risk communities. Preliminary data that the mortality rate for Black women in the facilities catchment area is twice that of other racial groups. Referral data shows fewer referrals to perinatologists. Focus groups with patients have raised several reasons, including health insurance coverage and poor patient-provider communication.

Initiative B: Reducing Overuse of Emergency Services by Mentally Ill Homeless Goal: Reduce use of emergency services by mentally Ill homeless by 50% by providing screening services and consultation with the Department of Psychiatric Services. Preliminary data show a 50% increase in walk-ins of mentally ill persons requesting medical care. This has caused disruption in the emergency department and confrontations with other patients and staff. These confrontations have led to the need for police intervention and heightened security. Patients who are not deemed in need of immediate medical stabilization sometimes return to facility grounds after being removed. Leadership recognizes that this situation could be a public relations disaster if not handled correctly.

Initiative C: Converting Pediatric Physical Therapy Center into a Center to Senior Wellness Center Goal: After the merger with a pediatric facility, leadership has decided to convert the Pediatric Physical Therapy into a Center to Senior Wellness Center A major goal to remain competitive has resulted in a merger with a pediatric medical center that includes a recently expanded, state-of-the-art pediatric physical therapy center. A data analysis of projected needs indicates that a senior wellness center offering preventive and mental health services, along with recreation would be profitable and fill an unmet need. Issues that need to be addressed include personnel, modification of the facility, outreach to the community, and referral for other services provided by the facility.
