This contract is between Employer Corp. and [Employee]. By signing at the bottom of this document, you understand and agree to the terms and conditions listed below: BENEFITS You will have access to benefits through Employer Corp.'s employee benefit group insurance. The benefit premiums are paid by Employer Corp., but participation in the employee benefit program can be revoked at any time. VACATION AND PERSONAL DAYS Vacation: During your first two (2) years of employment, you are entitled to two (2) weeks of vacation. After three (3) years of service and if you receive a satisfactory performance review, an additional week of vacation may be offered. Vacation time is paid at four percent (4%) of wages. Vacation time must be requested at least three (3) months in advance. Personal Days: You are entitled to two (2) paid and eight (8) unpaid personal days to be used for sickness or other reasons. Personal days must be requested at least two (2) weeks in advance. You must provide a doctor's note to prove your entitlement to use a personal day. HOURS Regular hours of work are from 9:00 a.m. to 5:00 p.m., Monday to Friday. A thirty (30) minute lunch period can be taken between 11:30 a.m. and 1:30 p.m. Two (2) fifteen (15) minutes breaks are provided, one in the morning and one in the afternoon. They cannot be taken consecutively, immediately before or after lunch, or at the end of the shift. If breaks are misused, they can be revoked at any time. RULES AND REGULATIONS Employer Corp.'s employees are the face of its business. Accordingly, you must dress and behave professionally at all times. Women are expected to wear clean, pressed knee-length skirts and blouses, and men must wear suits and ties at all times. Employees must not use smartphones, tablets or other devices during work hours, except during lunch and break times. Desks and offices must be kept clean and tidy. Eating at your work station is not permitted. You must not discuss any work-related business or information about Employer Corp. or its clients for one (1) year after your employment with Employer Corp. terminates. All computers and other devices in the workplace belong to Employer Corp., and should be used for Employer Corp. business only. No personal use is permitted, unless approved by your manager in advance. Employer Corp. reserves the right to monitor employees' email and Internet use. Employer Corp. has zero tolerance for the use of drugs (including marijuana) or alcohol at work. If you consume any drugs or alcohol at work, you will be dismissed immediately for cause.
