According to your text, what are the leading causes of most wrongful convictions? What has been done to attempt to remedy these causes of wrongful convictions?

In my experiences I see a lot of mistakes on the first level of the criminal justice system starting with the civilian police community when it comes to community policing. When the officer arrives on the scene to investigate if he fails to get a PID "positive identification" that is going to send him in a wrong direction, I saw this many time overseas. Where the Intel would be false and we go down the rabbit whole chasing a lead on a hard or soft target. I can't imagine what the civilian population is doing under a non-distress atmosphere. Lets say you do have PID you still must do a deep dive to weigh your next move, is it worth the taking of a person in to interrogate them to break them for information based on a false PID. When looking for a principle or targeted person separation of the witnesses is key, the reasoning for this is there will be no outside influences to discourage said witness for information.
