The Story of Jack Burns and Cecelia Langer Jack Burns, a 74-year-old retired cross-country truck driver, came to the emergency department of his hometown hospital because of “kidney problems” and dehydration. The health care team quickly decided that he should be admitted for further tests. He balked, saying he was “allergic” to hospitals, but after talking to the doctor in the emergency department, agreed to “stay overnight—that’s all.” Cecelia Langer, a nurse practitioner from the renal unit, came down to the emergency department and accompanied him to the admissions area because he seemed weak and a little disoriented. When the intake clerk presented him with the general hospital admissions informed consent form he retorted, “You got a noose around my neck. I know I gotta sign this thing to get treated.” Cecelia encouraged him to read it and went over the main points with him. Jack was admitted to the general medical unit because no beds were available in the renal unit. The next day, he was presented with another consent form by the medical unit nurse, this one for the tests themselves, and he became belligerent. He asked for “that nurse that helped me last night.” Cecelia was called from the renal unit and came into his room. He said he could not understand what they were trying to get him to do. She patiently explained the procedure he would undergo, referring to the informed consent form each step of the way. Although she herself thought it seemed quite well written, it took almost 20 minutes of talking with him before he decided to sign it. But when the imaging team took him to prepare him with contrast dye for the test, he again became agitated, claiming he had never been told that they were going to “inject poison” into him. The imaging team leader took out the form and showed him the place where it had been discussed. He still seemed disgruntled but just shrugged his shoulders and said, “Okay. But I hope that woman who explained it to me knows she did a terrible job because I would have never signed it.” The imaging team gave some consideration to postponing his test, but when they asked him if this is what he wanted, he grunted, “Go ahead! Do the test!” The team also thought about whether to tell Cecelia about his comment and decided to do so. At first she was annoyed, but later she found herself thinking about what to do to avoid this kind of thing happening to a patient again. Maybe he really did not understand!
